The State of Arizona has updated AZ Form 5000 and AZ Form 5000A, which are provided to vendors when claiming sales tax exemptions for University purchases. The partially pre-filled forms and instructions are now available on our Arizona Sales and Use Tax webpage. The changes include new numbering for commonly claimed exemptions due to recent updates to Arizona Revised Statute references.
We have also added an additional AZ Form 5000 to the Arizona Sales and Use Tax webpage regarding software accessed online, rented or purchased by a state university, and used to assess or test student learning or to promote curriculum design or enhancement. This form was added due to an increase in this exemption being applicable.
Please delete any copies of these forms that you have saved and download the latest version when needed to ensure vendors receive the most current information. Instructions for each form have also been updated to reflect these changes.
